Adult Patients
If you are an adult patient are awaiting surgery, one of my secretaries should be reaching out to you. A surgery counsellor at the Oakville location (phone number: 905 337 3736) can also help discern where you are on the wait list. For regular patients (who are not awaiting surgery), you may request a follow up either at the Oakville or TESS office. They will need to get the old records before booking, so please be patient. If you have not been seen since before October 2023, you will need another referral.
Kids
For kids, parents can request to be seen at either St Joseph’s Hospital or TESS. You will not be able to book you an appointment on the phone immediately until your request can be processed. The appointment may not be given to you for up to two weeks. If I have not seen you or your child in the last two years please request another consult request via their family doctors or optometrist to TESS, St Joseph's eye clinic or OTEC (adults only).
Requesting Medical Records
If you would like a copy of your chart from the Bloor office, then you must mail me a cheque for $40 to the attention of Dr M. Iizuka at 30 the Queensway, Toronto, Ontario, M6R 1B5. The cheque should be made out to Iizuka Medicine Professional Corporation Please indicate an email address where your chart can be sent electronically OR include a self addressed stamped envelope with the appropriate postage. Please be aware that electronic communication may not be secure if you send me a non confidential email address.
Finding a new eye doctor
There are many other ophthalmologists in the area. I would recommend you search for another doctor via the internet. Optometrists do not need a referral, and for many people, this visit may be covered by OHIP. For those of you who would like to see a specialist, you can also obtain a referral from your GP or optometrist to any number of ophthalmologists in the area.
